Here’s what to expect as care begins:

1 – A conversation – either in-person or by phone/email to learn all about how we’ll be able to help you and to grant your permission to begin receiving care

2 – Delivery of any medical equipment needed to keep you safe and comfortable

3 – If your loved is being discharged from a nursing home or hospital, we will work with that facility and keep you informed of the expected discharge date and estimated times of arrivals/deliveries and of any additional arrangements

4 – On the first day home (or the first day of care if already home), hospice medications will be delivered, and a nurse will visit to assess needs, advise as to what to expect while on hospice care and train your loved ones, family and caregivers how to achieve the best care. Throughout the first week of care, you will encounter additional hospice team members and you’ll receive a schedule of visits.
